Implementing a home transfusion program

A colleague in California reports that they have no procedures in place for offering 'home transfusions', but have received a request by a hospitalist physician to issue blood to him to take to the patient's home for transfusion. The inquiring collegue wonders if anyone is supplying blood for home transfusions, and if so, what are the issues to watch out for and how best to prepare?
