Association Bulletin #04-05:
Uniform Donor History Questionnaire

The American Association of Blood Banks (AABB) issued Association Bulletin #04-05 on June 29, 2004.  The purpose of this bulletin is to announce the availability of the new AABB Uniform Donor History Questionnaire (DHQ), which has been accepted by the Food and Drug Administration (FDA).  The collateral materials (donor education material, a medication deferral list, and a user brochure) and the questionnaire collectively provide all the information needed for qualifying a donor to give blood.  This bulletin supersedes and replaces the uniform donor history questionnaire in Association Bulletins #99-10 and #02-05.  Members are strongly encouraged to use these new materials.
